ImagineCare, an innovative provider of Digital Health Solutions, and Telia Company, are partnering to offer solutions for remote patient monitoring and individualized chronic care management. Initially, the partnership will target the Swedish market.

Connects and shares health data in real time
The ImagineCare solution allows patients to easily connect, measure their vitals and share health data in real time through a mobile app. Physicians, nurses and health coaches are connected to the platform to support the patient from a call-center managed by either the provider or ImagineCare. For the healthcare provider, working with ImagineCare improves the capabilities of interaction, follow-up and prevention. This increases patient engagement and improves care outcomes, resulting in reduced care costs and improved customer satisfaction. ImagineCare has been piloted by a number of healthcare providers and County Councils in Sweden since Q4 2017.
